PURPOSE - Under the general direction of the Revenue Cycle Director, the Medical Records Clerk ensures the medical record is complete in the Electronic Medical Record (EMR) by verifying all aspects of the record are scanned and signed. 

ENVIRONMENT - St. Vincent General Hospital is a combined 25-bed Acute Care Critical Access Hospital.  The Medical Record Clerk gathers patient information by collecting demographic information from a variety of sources; interacting with registration areas and physicians' offices; retrieving information from automated printer.  The working environment is generally pleasant, clean and well maintained.  The position requires a moderate amount of mobility.  The ability to reason, speak and have basic manual dexterity is essential.  Physical demands are moderate and may include sitting, standing for prolonged periods, walking, stooping and occasional lifting.  Psychological demands can be strenuous.  Most work is performed inside and the work is generally of a positive nature.

KNOWLEDGE AND EXPERIENCE - The Medical Records Clerk position requires the incumbent to possess the minimum of a high school diploma and one to two years experience in computerized hospital patient records is preferred. Medical Records Clerk must make sure patient information is entered accurately into the record and must be detailed-oriented. Time management, organization, attention to detail, quality, focus, professionalism, productivity, thoroughness, coordination, internal communications, reporting skills a must.  This position must present a professional appearance, deal courteously and respectfully with patients, be able to learn and follow established departmental policies, be able to perform work with attention to detail, be able to maintain accurate records and be able to contribute to a pleasant, positive working environment. Registered Health Information Technician (RHIT) Certification preferred, or obtained within six months of hire.

CHALLENGE - Prompt resolution of patient accounts is critical to the cash flow of this facility so every effort must be made to efficiently administer the accounts receivable cycle.

CONTACTS - The Medical Records Clerk may have contact with patients and their families, physicians and their office staff, other hospital staff members, and the general public.  Every effort should be made to project a professional, positive image of the department and the hospital when communicating with their contacts.
